中国已经步入人口老龄化时代,老年人在人群中所占比例日益增加。伴随老化而来的脑老化神经退行性疾病已经成为影响老年人健康的重要因素。其中阿尔茨海默病已经成为影响国民生活质量,阻碍社会发展的公共健康问题之一。人脑的动力学系统可以用非线性动力学系统来模拟,非线性动力学理论中的临界转变理论对找到功能临界点很有帮助。通过结合局部信息传递假说和临界转变的全脑动力学特征我们提出了依据静息态磁共振时间序列的基于顶点的功能临界指数方法(vertex-wise Index of Functional Criticality  ,vIFC)。本研究通过vIFC方法,找到脑老化神经退行性疾病——从主观认知下降(subjective cognitive decline, SCD)到遗忘型轻度认知损伤(amnestic mild cognitive impairment, aMCI)再到阿尔茨海默病(Alzheimer’s disease, AD)演化过程中的关键节点,并通过计算 vIFC与行为数据的相关证明该方法是否可以作为新的神经影像指标的证据。研究一使用42个SCD病人、67 个 aMCI 病人、34 个 AD 病人及 71 个正常人的结构功能磁共振成像数据及所有被试的性别,年龄,受教育程度等人口统计学信息,利用 vIFC 方法进行疾病和匹配的 NC 组的组间差异计算找到了 AD 进程中不同阶段的功能临界指标。研究二在研究一的基础上, 利用三组病人与NC组的vIFC 显著差异图谱与每组病人的简易精神状态检查表 MMSE(Minimum Mental State Examination),临床痴呆评定量表(Clinical dementia rating scale, CDR),听觉词语学习测验(auditory verbal learning test, AVLT),简易蒙特利尔认知评估量表(Montreal Cognitive Assessment,  MoCA)及其分量表等诊断量表分数以及性别,年龄,受教育程度等人口统计学信息等测量数据进行相关研究,找到了 vIFC 指标与行为数据的相关。本研究使用的基于顶点的功能临界指标方法为人脑成像研究从动力学的角度提供了新的方法,而且这种关于阿尔茨海默病的动力学轨迹的证实也为 AD的早期探查和干预提供了新视角。 

Other Abstract

China has stepped into an aging population,the elderly population is increasing rapidly in China, along with high incidence of neurodegenerative diseases, especially Alzheimer’s disease (AD), a significant public health issue that hinders economic and social development. Generally, human brain dynamics can be modeled as a nonlinear system, and the critical transition theory in nonlinear dynamics may be helpful for studying functional criticality. By combining both the local information transfer hypothesis and the global dynamical characteristics of  critical transition from nonlinear dynamics, we proposed a vertex-wise Index of Functional Criticality (vIFC) of a resting-state fMRI (rfMRI) time series in this study. In study one, we used the functional and structural MRI data of 42 subjective cognitive  decline (SCD), 67 amnestic mild cognitive impairment (aMCI) and 34 AD patients as well as their respectively age-, sex-, education level-matched normal control groups (NC), together with their scores and we found functional criticalities in the progression of AD. In study two, on the basis of study one, we used the vIFC maps of the three patients group and their scores of Minimum Mental State Examination, Clinical dementia rating scale, auditory verbal learning test as well as Montreal Cognitive Assessment  and their information about sex, age, years of education to do the correlation research, and we found the correlation between vIFC and   behavioral data. By combining nonlinear dynamics and rfMRI methodologies, our vIFC study has a solid theoretical basis  and could provide novel insights into human brain imaging studies from the dynamical perspective.
