承诺行动问卷信效度检验及在慢性疼痛患者中的初步应用

心理灵活性(Psychological Flexibility, PF)是接纳承诺疗法(Acceptance and Commitment Therapy, ACT )的核心概念。心理灵活性是指个体有意识地充分接 触当下,同时根据价值方向改变或坚持行为的能力,由六个相异但是又互相联系 的子过程组成:接纳、解离、以己为景、活在当下、价值以及承诺行动。承诺行 动在模型中具有方向性的指导作用。本研究聚焦于承诺行为(Committed Action )检验测量承诺问卷的(CAQ-8 )的信效度以及在慢性疼痛患者的应用,为中国进行承诺行动、心理灵活性和ACT的研究提供测量心理学指标的评估工具。

木研究分为两个子研究。研究一:接纳行动问卷(CAQ-8 )在大学生群中测评信度、效度。以方便取样的方式发布问卷682份,回收了有效问卷590份。用SPSS 22. 0和Amos 22. 0进行数据分析。将数据随机分成两组,用主成分分析法对一组数据(n=320)进行探索式因素分析,另一组数据(n=270)进行验证式因素分析;采Pearson相关分析CAQ-8的效标效度和聚合效度,用层次回归载荷在0. 81-0. 92之间;验证性因素分析显示两个因子结构拟合较好(x²df =2.34,CFI=0.999 TLI=0.98, RMSEA=0.07); CAQ-8总分与AAQ-II, CFQ, PPS, DASS-21总分均呈现负相关(r=-0. 22--0.369 P<0.001)与MAAS呈正相关(r=0.20, P<0. 001)。控制AAQ-II, CFQ, MAAS, CAQ-8对焦虑、压力的解释力仍显著(△R²=0.30-0.39, P<0.001)。该量表和各分量的Cronbach 's a系数为0. 58,0.93、0.87。

研究二:接纳行动问卷(CAQ-8)中文版在慢性疼痛人群中测评信效度及应用。以方便抽样的方式发布795份问卷,共收回了263有效问卷,用SPSS 22. 0和Amos 22. 0进行数据分析。通过Pearson相关分析CAQ-8的效标效度度和聚合效度;用Pearson相关分析和单因素方差分析不同组承诺行动得分差异:用逐步多元线性回归分析承诺行动的影响因素。研究结果:CAQ-8总分与AAQ-I I , PPS, HAD总分、PCS总分均呈负相关(r=-0. 25--0.39,P<0. 001),慢性疼痛患者焦虑、抑郁高低分组承诺行动水平差异显著(t=4.55、4.65,P<0.01),慢性疼痛患者不同疼痛时间、疼痛部位数量组承诺行动得分差异不显著(F=1 .45、1.10, P> 0. 05),疼痛对饮食有无影响组承诺行动得分差异显著(t=2.14,P<0.05),疼痛对睡眠有无影响组承诺行动得分差异不显著(t=0. 54, P > 0. 05 )。自变量(HAD抑郁、CPAQ-8 , PPS、外伤性损伤)能解释因变量(慢性疼痛患者的承诺行动水平)变异量23.00%,慢性疼痛接纳程度对承诺行动影响最为显著。

本研究可以得出如下结论:

第一,中文版CAQ-8在大学生群体中具有良好的信度和效度,为中国背景下进行承诺行动的评测提供了良好的测量工具,可以用于心理灵活性和ACT的相关研究,并且提供了有效的关于承诺行动的测量工具。

第二,慢性疼痛患者的承诺行动受慢性疼痛接纳程度、拖延水平、抑郁水平以及外伤性损伤造成的疼痛等因素影响。

Psychological Flexibility (PF) is the core concept of Acceptance and Commitment Therapy (ACT). Psychological flexibility refers to the ability of individual to consciously and fully contact the present, while changing or adhering behavior according to the valuing, consisting of six distinct but interrelated sub-processes: acceptance, cognitive defusion, self as context, being persent, valuing and committed action. Committed action as a directional guiding role in the model. This study focused on the core concept of committed action testing the reliability and validity of the Committed Action Questionnaire (CAQ-8) and premliary application in chronic pain to provide a good assessment tool for measuring psychometric indicators for China's committed action, psychological flexibility and ACT research.

This study is divided into two sub-studies. study 1 to measures reliability and validity of Committed Action Questionnaire (CAQ-8) in the college students. 682 questionnaires were pulished in a convenient sampling manner 590 valid questionnaires were collected. Data analysis by SPSS 22.0 and Amos 22.0. The valid questionnaires were randomly divided into two groups, of which one subset (n=320) was used for exploratory factor analysis (EFA) and the other (n=270) for confirmatory factor analysis (CFA).Criterion and convergent validity were tested by Pearson correlation respectively. Incremental validity was tested by hierarchical regression analysis. The EFA suggested that factors with explained 7'7.09% of the total variance and the factor loading ranged from 0.81 to 0.92, confirmed the hypothesized two-factors mode (x²/df=2.34, CFI=0.99, TLI=0.98, RMSEA=0.07),CAQ-8 total scores were negatively correlated with AAQ-II, CFQ, PPS, DASS-21 total scores(r=-0.22 to-0.36, P<0.001), significant positively correlated with MAAS (r=-0.20, P<0.001),the explanatory power of AAQ-lI, CFQ, MAAS and CAQ-8 on anxiety and stress was still significant (△R²=0. 30 to 0. 39; P<0.001), the Cronbach alpha coefficient of the scale and each component 0.58, 0.93 and 0.87.

Study 2 to measures the reliability and validity of Committed Action Questionnaire (CAQ-8) and premliary application in chronic pain. 795 questionnaires were published in a convenient sampling manner and 263 valid questionnaires were collected. Data analysis by SPSS 22.0 and Amos 22.0. Criterion and convergent validity were tessted by Pearson correlation respectively. Analyze the differences in comnutted action scores of different groups by Pearson correlation and one-way ANOVA. Analyze the influencing factors of committed action using stepwise multiple linear regression. CAQ-8 total scores are significant negative correlation with AAQ-II, PPS, HAD total scores(R=-0.25 to -0.39, P<0.001),there was a significant difference in committed action scores between the anxiety and depression high and low groups in chronic pain (t=4.55, 4.65, P<0.01), there was no significant difference in committed action scores between pain time groups and pain sites number groups in chronic pain (F=1.45, 1.10, p >0.05),there was a significant difference in committed action scores between pain affect diet and not groups (t=2.14, P<0.05), there was no significant difference in committed action scores between pain offect sleep and not groups (t=0.54, P>0.05), HAD depression, CPAQ-8,PPS, traumatic injury as independent variable explain the change of the dependent variable committed action in chronic pain by 23.00% and the degree of chronic pain acceptance was the most affectted committed action.

The conclusion are as follows:

First, the Chinese version of CAQ-8 has good reliability and validity in the college students and provides a good tool for the evaluation of committed action in China. Related research that can be used for psychological flexibility and ACT provides an effective measurement tool.

Second, the committed action of chronic pain are affected by clu-onic pain a cceptance, procrastination, depression and pain factor caused by traumatic injury.
