目的调查不孕症患者创伤后应激障碍(PTSD)现状,并分析生育压力、正念水平及PTSD间的关系。方法采用方便抽样的方法选取山东省某生殖医学中心不孕不育夫妇中的女性362名为被试,使用生育相关压力量表、正念水平量表及创伤后应激障碍量表-平民版进行问卷调查,应用回归分析对正念的中介及调节作用进行检验。结果不孕症患者创伤后应激障碍得分为(31.27±11.68)分,其中≥38分者占24.3%。分层回归分析显示,正念在不孕症患者生育压力和创伤后应激障碍间存在中介作用,其中介效应为0.188,占总效应的33.04%;且正念在不孕症患者生育压力和创伤后应激障碍间存在调节作用。结论生育压力可以直接影响不孕症患者的PTSD症状,还可以通过正念间接影响PTSD;正念水平能够缓冲生育压力对PTSD的影响,对不孕症患者的PTSD的发生具有保护作用。

To investigate the influencing factors of posttraumatic stress disorder ( PTSD) in infertile women and to evaluate the protective effect of mindfulness on the relationship between infertility stress and PTSD．Methods Totally 362 patients in a reproductive center in Shandong Province were enrolled and surveyed with Fertility Problem Inventory ( FPI) ，Mindful Attention Awareness Scale ( MAAS) ，PTSD Cheeklist-Civilian Version． The mediating effect and moderating effect of mindfulness were analyzed with hierarchical regression analysis． Ｒesults The average score of PTSD of infertile women was 31． 27 ± 11． 68，and 24． 2% of patients had score ≥38． Logistic regression analysis showed that mindfulness partially mediated the effect of infertility stress on PTSD，with a mediating effect of 0． 188，accounting for 33． 04% of the total effect． Mindfulness also moderated the association between infertility stress and PTSD． Conclusion Infertility stress has both direct and indirect effects via mindfulness on PTSD． Mindfulness buffers the effect of infertility stress on PTSD and is a protective factor of PTSD in infertile women．
